The evaluation forms below are designed as WORD templates that you may open, complete, save (for future reference), print, and sign. Completed forms should be reviewed with your student intern and they are responsible for turning it in to their assigned University supervisor.

Cooperating Principals

Thank you for welcoming our Northwest University interns into your building. When the interns begin their full-time student teaching, please schedule a time (at your convenience) to observe them teaching a class. You may use the Student Teacher Evaluation form or the Pedagogy Assessment form as a guideline for your observation, if you desire.

Please submit a letter of reference for each of this semester’s full-time student teacher’s placement files. The letter should be sealed and sent directly to: Northwest University School of Education, P.O. Box 579, Kirkland, WA 98083.

We would also greatly appreciate your time to fill out the Northwest University Program Assessment below and submitting to our office with the letter(s) of reference. These assessment are an essential part of our program improvement plan.

The link below is to the state required Peformance-based Pedagogy Assessment form that is administered at least twice by our University supervisors for each student teacher. The administration is required for certification. It is provided here simply for information purposes. You may choose to use it as a part of your evaluation of the student teacher or as a self-assessment tool for them or not at all.
