Junior Cohort Information
To retain your place in the Cohort this documentation must be turned in by the dates given below.
Immunization Record
You will need to have all of the immunizations listed on this form and have all the titers listed drawn, with the appropriate signatures and information included as well. If you have questions about the requirements of this form, be certain to ask the office well in advance.
The immunization record must be complete and turned in to the Buntain School of Nursing office by August 1, 2011.
Medical Clearance Form
Your doctor should provide your physical information on the Medical Clearance Form. If you have questions about the requirements on this form, be certain to ask the office well in advance.

Proof of Insurance
You must provide the Buntain School of Nursing office with a copy of your medical insurance card by August 1, 2011.
CPR Certification
Provider Level CPR certification is required of each student. A CPR certification course will be offered Friday, August 26, 2011, from 1:00pm to 5:00pm in HSC-233. The course fee of $50 is due at the beginning of class. Cash or check only (checks made out to Merit Emergency.)
If you do not sign up for this course you must provide a photocopy of your Provider Level CPR certification prior toAugust 1, 2011.
Passport
Participation in a four-week, cross-cultural nursing and ministry experience in the senior year is an essential element of the Buntain School of Nursing curriculum for which a valid passport is needed. You must have a passport in your name that expires no earlier than October 1, 2013
The School of Nursing will need a photocopy of your passport by August 1, 2011.
Orientation
Orientation for incoming Junior nursing students will take place Friday, August 26, 2011, from 10:00am to 12:00pm. This is required for all Junior nursing students.

Save the Date – Consecration Service
A Consecration Service will be held on Tuesday, September 27, 2011, in the Butterfield Chapel at 7:00pm. Please plan to invite your family and friends to this special event! This is a time for the new class of nursing students to be dedicated and set apart for the ministry to which God has called them and to corporately commit themselves to His service. A reception will follow.
