Junior Cohort Information
Requirements & FAQ
The documentation listed on this page must be turned in between July 25, 2016 and July 29, 2016, to retain your place in the 2016 cohort. All documentation must be mailed to the Buntain School of Nursing, Northwest University, P.O. Box 579, Kirkland, WA 98083. An envelope has been included in your admissions packet for this purpose.

Along with your admissions letter an orientation packet was mailed to you. It included the following documents. You will need to follow the instructions for each immunization listed below. Once you have obtained the correct documentation for each immunization you are ready to submit the forms follow the instructions stated above in the section titled Immunization Review.
- Hepatitis A
- Hepatitis B
- TB (PPD or QuantiFERON-Gold)
- TB Symptom Check (Positive TB test only)
- Varicella (Chicken Pox)
- Polio Series
- Flu (September or October of your Junior & Senior year)
Medical Clearance Form
Your doctor should provide your physical information on the Medical Clearance Form. If you have questions about the requirements on this form, be certain to ask the office as soon as possible.
The Medical Clearance Form must be complete, signed by your primary care provider and turned in with the other immunization documents.
Additional Required Documents
You also need to present the following documents with the other immunization documents.
- Criminal Background Check and Disclosure Form
- Health Insurance Card
- Licensure (RN, LPN, CNA, Tech.)
- US Passport (Expiration after October 15, 2018)
- Drivers License (must be valid)
All Buntain School of Nursing students are required to take the AHA Provider Level CPR certification class at NU. You may choose from the following times:
- Saturday, August 27, 2016, 8:00 a.m. to 12:00 p.m.
- Saturday, August 27, 2016, 1:00 p.m. to 5:00 p.m.
Each session will accommodated approximately 22 students. If we fill up these two class times we will offer a 3rd option on Friday evening. The cost is $60.00, cash or check made out to Merit Emergency. You may sign-up for the day and time you prefer at the following link:

Orientation for incoming Junior nursing students will take place Friday, August 26, 2016, from 8:30 am to 4:00 pm. This is required for all Junior nursing students.

Please download these forms, complete them, and bring them with you to orientation on Friday, August 26th.
Save the Date – Consecration Service
A Consecration Service will be held on Tuesday, September 27, 2016, in the Butterfield Chapel at 7:00 pm. Please plan to invite your family and friends to this special event! This is a time for the new class of nursing students to be dedicated and set apart for the ministry to which God has called them and to corporately commit themselves to His service. A reception will follow.
- Weeks 1-4 (August 29 – September 23, 2016)
- Week 5-15 (September 26 – December 12, 2016)
- Juniors Finals Schedule (December 13 – December 16, 2016)
- Note: the prices listed are from 2013. Acorn Uniform will have new prices.
